Lanai Community Health Center would like for you to get to know your Family Health History (FHH). Knowing your FHH can save your life and the life of those you love. It can help detect, manage and prevent diseases and or conditions before it ever happens.

The Family Health History Tool helps you to:

  • Identify trends & patterns of disease that may lead to treatment or prevention.
  • Captures heredity, diet and environment in one location.
  • Increases health and genetics knowledge for the individual and the family.
  • Promotes conversations about health in the family and community.

FHH is the story of disease that runs in your family. It is one part of the entire history of your family. Along with culture, values, environment, and behaviors, FHH influences the way you live your life.

Learning about your FHH can help you make healthy choices: it is a cheap, easy way to improve your own health and the health of your family. Share the information you gather with your healthcare provider to further reduce your risk of disease and create a partnership around your health.

It won’t take that long. And who knows? Your actions could be a life saving moment for you and your family. Below are two downloadable PDF books that will help you understand the process and get started.

Once you have reviewed the downloadable PDF toolkit books, go to:
https://familyhistory.hhs.gov/fhh-web/familyhistory/start.action to enter your family’s information. When you are done, you will end up with a Genetic Pedigree (see sample PDF below the two toolkit books) that can be used for conversations with your family and provider in helping you make healthy choices.
